Remember to complete the forms, attach proof of payment (cancelled cheques) and a sample of the Kitec pipe that was removed from your suite -- you would have received the pipe sample at the time of removal.
(Please note that the common practice for Kitec removal involved the remediation contactor leaving the pipe samples in each suite on the day of removal. Regrettably, if you have misplaced your sample, it is very unlikely that additional samples will be available.)
